FDLRS Island Coast Child Find works in partnership with the Collier and Lee County Public Schools to assist in the identification and screening of children ages 3 through 5 who are not enrolled in the public school system and who have or are at risk of developing disabilities or learning problems. Submission of this form serves as a request for a return phone call to schedule a screening.
